community.Hours:Monday through Thursday : 9:00 am - 5:00

provide high-quality, patient-centered care to adult patients in a
fast-paced environment. This role involves comprehensive patient
management, including diagnosis, education, treatment planning, and
ordering of pertinent tests, therapeutics, and medications. The
physician will collaborate with the clinical team to deliver
coordinated care and maintain accurate documentation within the
electronic medical record system in accordance with Aizer Health's
policies.Key ResponsibilitiesProvide high-quality care by
accurately diagnosing and treating patients with acute and chronic
illnesses.Maintain appropriate documentation within the electronic
medical record system in line with Aizer Health's policies.Improve
gaps in care, enhance care coordination, and increase patient
satisfaction.Order appropriate lab and imaging tests with timely
follow-up on treatment plans to reduce morbidity, hospitalization,
and mortality.Make appropriate referrals to specialists for
tertiary care when indicated.Collaborate effectively with
multidisciplinary team members to ensure comprehensive patient
care.QualificationsDoctor of Medicine (MD) or Doctor of Osteopathic
Medicine (DO) degree from an accredited medical school.Completion
of an accredited residency program in primary care or related
field.New York State Medical Licensure or eligibility to obtain
one.DEA license or the ability to obtain one after hire.Pay &
